Job Description:
Provide safe, timely and accountable pharmaceutical services across the hospital: manage procurement, storage and dispensing of medicines (with emphasis on paediatric and perioperative/ICU medications), deliver ward- and ICU-based clinical pharmacy support, lead medication-safety and stewardship activities, and maintain pharmacy governance, documentation and staff competency.
Key responsibilities
1. Dispense and verify medicines for wards, ICU, Cath-Lab and OT: check dose, route, frequency, interactions and paediatric/weight-based calculations before supply.
2. Provide clinical pharmacy on rounds: perform medication reconciliation on admission/discharge, review charts, recommend dosing adjustments (renal/hepatic/weight), and advise on perioperative medication plans (antibiotics, anticoagulation, inotropes).
3. Prepare and supervise aseptic/sterile IV admixtures and TPN (where delegated): ensure SOP adherence, correct labelling, compatibility checks and documentation.
4. Lead formulary management and therapeutic substitution: maintain approved drug lists, support antibiotic stewardship, anticoagulation protocols, and update medication-use policies with clinical leads.
5. Manage procurement and inventory: forecast demand, place orders, monitor expiries, rotate stock, maintain cold-chain and controlled-drug logs, and ensure secure storage of high-value/temperature-sensitive items.
6. Ensure controlled-substance security and recordkeeping: perform daily counts, maintain narcotics logs, reconcile discrepancies and report immediately per policy.
7. Implement medication-safety systems: report and investigate medication incidents, run root-cause reviews, implement corrective actions and conduct regular medication-use audits.
8. Provide drug-information and education: advise clinicians, nurses and patients/families on dosing, interactions, administration and adverse effects; support CPD for pharmacy staff.
9. Maintain accurate pharmacy records and HMIS entries in English: dispensing logs, procurement records, inventory reports and financial documentation for audits and donor reporting.
10. Participate in on-call and emergency medication support for perioperative/ICU needs (urgent drug kits, blood-product related medications) and contribute to procurement specifications for high-cost medicines.
Job Requirements:
· Four-year Bachelor degree in Pharmacy (BPharm, DPharm or equivalent) with current national registration/license and eligibility to practice in Afghanistan.
· Certificate of internship or practical hospital pharmacy training where required by local regulation.
Preferred / desirable:
· Postgraduate diploma or training in Clinical Pharmacy, Hospital Pharmacy, Critical Care Pharmacy or Pharmacovigilance.
· Formal training/certificate in aseptic compounding, TPN preparation, antimicrobial stewardship or cold-chain management.
Minimum experience
· Minimum 4 years’ hospital pharmacy experience; at least 12 months in a tertiary/ICU or critical-care pharmacy setting preferred.
· Demonstrated experience in procurement, inventory control, clinical pharmacy interventions and handling of temperature-sensitive or high-cost medicines.
